The spot formula is a magistral preparation combining ivermectin 1%, metronidazole 0.75% and niacinamide 2% in one gel, made by a compounding pharmacy after the prescription is issued. Ivermectin 1% cream and metronidazole 0.75% are each authorised in the EU for the papules and pustules of rosacea.

In the pivotal 12-week trials of ivermectin 1% cream alone, inflammatory lesions fell by about three quarters, and in a 16-week head-to-head trial ivermectin was more effective than metronidazole 0.75%. The compounded combination itself has not been studied as a product; a licensed doctor weighs this before prescribing.
