What happens if I stop taking tirzepatide?

Weight tends to come back. In SURMOUNT-4, adults who had already lost a mean of 20.9% over a 36-week lead-in were randomly assigned either to keep taking tirzepatide or to switch to placebo. Between week 36 and week 88 the continued group changed by a further -5.5%, while the placebo group regained 14.0%. At week 88, 89.5% of the continued group had kept at least 80% of the weight lost during the lead-in, compared with 16.6% of the placebo group. Stopping is a decision to make with your prescriber.

Does everyone have to go up to the 15 mg dose?

No. The product information lists 5 mg, 10 mg and 15 mg as the recommended maintenance doses, so 5 mg and 10 mg are licensed end points in their own right and not only steps on the way up. In SURMOUNT-1 the 5 mg group had a mean body-weight change of -15.0% at 72 weeks, the 10 mg group -19.5% and the 15 mg group -20.9%.
