DERMATOLOGY

Facial hyperpigmentation

The melasma pathway assesses any facial hyperpigmentation. The doctor confirms whether it is melasma or a post-inflammatory mark, prescribes a depigmenting course when appropriate and refers lesions that need examination to in-person care.

WHAT IT IS

Facial hyperpigmentation is any localised darkening of facial skin from excess melanin. The most common causes are melasma, marks left after acne or inflammation (post-inflammatory hyperpigmentation) and solar lentigines.

Telling them apart matters because they are not all treated the same way, and some pigmented lesions, especially single, asymmetrical or changing ones, must be examined in person to rule out other diagnoses.

In the online consultation, the doctor examines your photos, identifies the type of mark and, when it is melasma or a treatable post-inflammatory hyperpigmentation, prescribes a supervised depigmenting course with strict sun protection.

HOW COMMON IT IS

Very common in adults, particularly in medium and darker skin types and after acne; melasma is the most frequent cause of symmetrical patches in women.

CAUSES

  • Melasma (hormonal and sun-driven)
  • Post-inflammatory hyperpigmentation after acne, eczema or injuries
  • Solar lentigines from cumulative exposure
  • Photosensitising medicines
  • Cosmetics or fragrances that react with sunlight

TREATMENT APPROACHES

  • Daily SPF 50+ sun protection as the first measure in every case
  • Hydroquinone 4% cream or gel, only on the patches, in a limited supervised course
  • Kligman-type compounded formula for selected melasma
  • Treating active acne to prevent new marks
  • Review with photos at the end of the course
  • Laser, peels or assessment of doubtful lesions only in person

WHEN TO SEE A DOCTOR

See a doctor if the patches do not improve with sun protection or worry you. A single, asymmetrical, raised, bleeding or changing spot must be examined in person without delay.
