What Qsymia is, and how to get treatment

Qsymia is the United States brand name for a capsule that combines phentermine and topiramate; it was approved there in July 2012. In Europe the same product is called Qsiva, and its history is complicated. There is no EU-wide approval: the European Medicines Agency refused the Europe-wide application in 2013 because of concerns about long-term effects on the heart, on mood and on memory and attention. The manufacturer later obtained national approvals through a separate route led by Sweden, and the product is on the market in a small number of northern European countries — Sweden, Denmark, Finland and Poland, plus Norway and Iceland. In most EU countries, including Spain, it is not authorised, so a doctor there cannot prescribe it.

Qsymia has no EU marketing authorisation for this use. It may be an approved medicine for something else, or approved in another country — but not something a doctor here can prescribe for this.

CONQUER ran for 56 weeks in 93 centres in the United States only; when the European Medicines Agency reviewed this programme it judged that a cardiovascular outcomes trial would be needed before it could accept the long-term safety of the combination, and that trial has not been published.

Depression and anxiety

Depression-related events in 7% on the higher dose versus 4% on placebo; anxiety-related events in 8% versus 3% on placebo. The EMA specifically cited long-term psychiatric effects from topiramate when it refused the EU-wide application.

Effects on the heart and circulation

The EMA's committee refused the EU-wide application partly because phentermine is known to increase heart rate and its long-term effects on the heart and blood vessels are not clear.

Risk in pregnancy

Topiramate is known to cause birth defects and has been the subject of an EU safety referral; this combination must not be used in pregnancy and effective contraception is required.
