What Naltrexone is, and how to get treatment

Naltrexone blocks the opioid receptors in the brain. In EU countries it is authorised, as part of a wider treatment programme, to help people with alcohol dependence and people who have come off opioids and want to stay off them. "Low-dose naltrexone" (LDN), taken at a fraction of the licensed dose for pain or fatigue conditions, is not an authorised medicine anywhere in the EU — it is compounded and used off-label. Naltrexone was tested alongside structured medical management and counselling, not on its own; the size of the benefit was modest and depended on that surrounding support.

Must be opioid-free before starting

Taking naltrexone while opioids are still in the body precipitates acute withdrawal, so a documented opioid-free interval and supervision are required before the first dose.

Blocks opioid pain relief

While on naltrexone, opioid painkillers will not work normally — this matters in an accident or emergency, and patients are advised to carry a warning card.

Common side effects

Nausea, headache, dizziness, anxiety and difficulty sleeping.

Liver

Liver function is monitored; naltrexone is not used in acute hepatitis or liver failure.

Low-dose naltrexone (LDN) is not an authorised product

LDN — roughly a tenth of the licensed dose, taken for pain, fatigue or autoimmune conditions — has no EU marketing authorisation. It is compounded and used entirely off-label, and systematic reviews of it in fibromyalgia report mixed results from small trials rather than regulatory-grade evidence.
