IN PLAIN TERMS

What Caverject is, and how to get treatment

Alprostadil is a copy of prostaglandin E1, a substance the body makes naturally that widens blood vessels. Unlike the tablets, it is applied straight to the penis - as a cream on the tip, as an injection into the side of the shaft, or as a small pellet placed into the urethra - so it acts on the blood vessels directly. It has been authorised in EU countries for erectile dysfunction since 1994 as an injection and since 2014 as a cream. Because it works locally, the main side effects are also local: burning, pain or redness where it is applied, and rarely an erection that lasts too long, which needs urgent medical attention.

Caverject has been authorised for use in the EU since 29 November 1994, so a licensed doctor can prescribe it when it is clinically appropriate for you.

NAMED SAFETY SIGNALS

What has been reported with Caverject.

Application-site burning, penile pain and redness (cream)

Penile burning, penile pain, penile erythema, genital pain and genital discomfort are all listed as common in the EU product information for alprostadil 3 mg/g cream. In the clinical trial the withdrawal rate was 30% in the 300 microgram group compared with 20% in the 200 microgram group.

Injection-site pain (intracavernosal)

Penile pain is listed as very common in the EU product information for alprostadil injection. In the six-month self-injection study, usually mild penile pain occurred in 50% of men at some time but after only 11% of injections.

Prolonged erection and priapism (intracavernosal)

In the EU product information for alprostadil injection, a prolonged erection lasting 4 to 6 hours developed in 4% of patients and priapism (a painful erection lasting more than 6 hours) in 0.4%. In the six-month self-injection study, prolonged erections occurred in 5% of men and priapism in 1%.

Prolonged erection and priapism (cream)

Listed as uncommon in the EU product information for alprostadil cream. Priapism was seen in 1 patient (0.06%) across the two 3-month studies and in 5 patients (0.4%) in the study lasting more than 6 months. An erection lasting more than 4 hours needs immediate medical help, because untreated priapism can permanently damage erectile tissue.

Penile fibrosis (intracavernosal)

Penile fibrosis, including angulation, fibrotic nodules and Peyronie's disease, was reported in 3% of clinical-trial patients in the EU product information for alprostadil injection, rising to about 8% in a self-administration study of up to 18 months. Injection-site haematoma was reported by 3% and bruising by 2%.

Dizziness and fainting

In the phase 3 cream studies, symptomatic low blood pressure (dizziness) and fainting occurred in 0.4%, 0.4% and 0.5% of patients on the 100, 200 and 300 microgram doses. Patients are advised not to drive or do hazardous tasks until they know how they react.

Effects on a female partner

Vulvovaginal burning and vaginitis are listed as common reactions in female partners in the EU product information for alprostadil cream.

Penile pain (intraurethral)

In the transurethral pivotal trial, mild penile pain was the most common side effect, occurring after 10.8% of treatments; in-clinic low blood pressure occurred in 3.3% of men, and no man had priapism or penile fibrosis.

About

What is Caverject?

Active ingredient: Alprostadil · Dosages: Cream 3 mg/g on demand

Alprostadil is a prostaglandin E1 that directly dilates the blood vessels of the penis. It's considered when oral PDE5 inhibitors are ineffective or contraindicated.

It's administered as a urethral cream or an intracavernous injection, under medical instruction — each route has its own efficacy and tolerability profile. Correct technique matters, and the doctor explains the protocol and the warning signs.

Safety first

Safety, in full.

Caverject is an effective medicine — and like any medicine it carries risks. Your doctor screens for all of this before prescribing. Here's the honest picture, not just reassurance.

Erectile dysfunction in adult men, as a local alternative to tablets.

  • Apply the single-use dose to the opening at the tip of the penis 5–30 minutes before sex, following the leaflet's technique.
  • The effect lasts around 1–2 hours.
  • Maximum one dose per 24 hours, and no more than 2–3 doses per week.
  • Wash hands after applying.

Who should not take it

  • Conditions that predispose to prolonged erections — sickle cell disease or trait, leukaemia, multiple myeloma.
  • An abnormally shaped penis, a penile implant, or severe scarring (Peyronie's disease) — discuss with the doctor first.
  • Men for whom sexual activity is inadvisable, for example because of severe heart disease.
  • Allergy to alprostadil or any ingredient of the cream.
  • If your partner is pregnant or breastfeeding, use a condom during sex.
