What Tesamorelin is, and how to get treatment

Tesamorelin is a laboratory-made copy of growth-hormone-releasing hormone. It is approved in the United States, under the brand name Egrifta, for one narrow purpose: reducing excess abdominal fat in people with HIV who have lipodystrophy. It is not authorised in the European Union — the application was withdrawn in 2012 after the EMA's expert committee said it could not have been approved, citing a rise in IGF-1 as a major safety concern and doubt that the fat reduction brought a real health benefit — so no doctor here can prescribe it. It is not a general weight-loss medicine, and the vials sold by online peptide sellers are unlicensed material whose contents cannot be verified.

This was studied only in adults with HIV on antiretroviral therapy who had excess abdominal fat; when the EU application was assessed, the CHMP said the fat reduction had not been shown to be clinically meaningful in terms of actual health benefits, and that the trial participants were not representative of European HIV patients.

What has been reported with Tesamorelin.

Rise in IGF-1 was the EMA's main safety concern

The CHMP noted "an increase in the level of a protein called insulin-like growth factor 1 (IGF-1) in a considerable number of patients", that "high levels of IGF-1 may be associated with an increased risk of cancer and a potential worsening of diabetic eye disease", and considered this "a major safety concern".

No long-term safety data

The CHMP recorded that "no long-term safety data were provided, although treatment with Egrifta was expected to be long term", and concluded that the benefits did not outweigh the risks.

Withdrawals for adverse events in the pivotal trial

In the 26-week phase 3 trial, overall adverse events did not differ significantly between groups, but more participants on tesamorelin than on placebo withdrew because of an adverse event.

Not a general weight-loss medicine

The approved US indication is reduction of excess visceral abdominal fat in people with HIV who have lipodystrophy. It has not been shown to work for general weight loss, and the CHMP specifically raised the difficulty of distinguishing lipodystrophy from ordinary obesity.

Unregulated supply

Tesamorelin sold by online peptide sellers is unlicensed material made outside medicines manufacturing rules, so its strength, purity and contents are unverifiable.
