Spironolactone is a water tablet. It is authorised in EU countries including Spain to treat high blood pressure, some types of heart failure, and conditions where the body makes too much of the hormone aldosterone. It also has a weak anti-male-hormone effect, and because of that some doctors prescribe it off-label for female pattern hair loss - off-label means outside the uses it was officially approved for. Hair loss is not an authorised use of spironolactone anywhere in the EU, and it is not something Hi-Doctor prescribes.

How it works

Spironolactone blocks the hormone aldosterone in the kidney so the body passes more salt and water in urine, and it also partly blocks the effect of male hormones on body tissue.

The five pooled studies (413 patients) were mostly observational, used doses from 25 to 200 mg and durations from six months to over a year, and none compared spironolactone against a placebo, so these figures cannot be separated from natural variation or from other treatments taken at the same time.

Scalp itching or flaking

Reported in 18.9% of women in the pooled analysis of five studies (413 patients).

Menstrual changes

Reported in 11.9% of women in the same pooled analysis; the Spanish product information lists irregular menstruation, absent periods and post-menopausal bleeding as adverse reactions.

Facial hair growth

Reported in 6.9% of women in the same pooled analysis.

Raised blood potassium

The Spanish product information requires potassium and creatinine to be checked in the week after starting or increasing the dose, then monthly for three months, quarterly for a year and every six months thereafter, and says treatment must be interrupted if potassium exceeds 5 mmol/L.

Breast tenderness and breast enlargement in men

Gynaecomastia and breast pain in men and women are listed as adverse reactions in the Spanish product information.

Pregnancy

The Spanish product information states spironolactone should not be used during pregnancy unless the potential benefit justifies the potential risk, and records reproductive toxicity in animals linked to its anti-androgen effect.
