Melanotan II is a laboratory-made peptide that switches on melanocortin receptors, the signals that make skin produce pigment. It is sold, usually as an injection or a nasal spray, to darken the skin. It is not an authorised medicine anywhere in the European Union and it is not a cosmetic, so no doctor can prescribe it — several European medicines agencies, including Ireland's HPRA and the Danish Medicines Agency, have published warnings telling people to stop using it. When vials bought online were analysed in a laboratory they contained less melanotan II than the label claimed, plus unidentified impurities, so what is in them cannot be verified.

NAMED SAFETY SIGNALS
The US Food and Drug Administration records that "published case reports discuss serious adverse events including melanoma, posterior reversible encephalopathy syndrome, sympathomimetic toxidrome and priapism".
The Health Products Regulatory Authority states that Melanotan 2 "is not authorised by the HPRA or any medicines regulator to treat any condition", names side effects including "the development of new moles, darkening of existing moles and freckles, potential loss of vision, muscle tremors, stroke and anaphylaxis", and advises anyone using it "to stop immediately".
The Danish Medicines Agency warns that Melanotan "is not authorised in Denmark, Europe or the United States", that "the effect of the product is not documented and there are no studies of the product's potential side effects", and urges users to stop and speak to a doctor.
An analysis of melanotan II vials purchased from three online shops found 4.32 to 8.84 mg of melanotan II in vials all labelled as containing 10 mg, and unidentified impurities of 4.1 to 5.9% in vials from two of the shops.
Dermatology case reports describe new and changing pigmented lesions, and melanoma, in people who have used melanotan. Case reports show association, not proof of cause, but they are the reason regulators advise stopping.
It is normally self-injected with equipment obtained outside any clinical setting, adding the risks of non-sterile injection and shared needles.
