Isotretinoin — widely known by the brand name Roaccutane, and as Accutane outside the EU — is a vitamin-A-derived capsule authorised in EU countries for severe acne: the nodular or scarring kind that has not cleared with antibiotics. It works by shrinking the skin's oil glands. It causes severe birth defects, so in the EU anyone who could become pregnant can only take it inside a formal Pregnancy Prevention Programme, with pregnancy tests before, during and after treatment and effective contraception throughout. This landmark study was very small (14 patients) and uncontrolled; modern EU practice uses lower doses, reserves isotretinoin for severe acne that has not responded to antibiotics, and requires dermatology supervision.

NAMED SAFETY SIGNALS
Isotretinoin is highly teratogenic and must never be taken during pregnancy. In the EU it may only be given to people who can become pregnant under a Pregnancy Prevention Programme: pregnancy tests before, during and after stopping treatment, at least one effective method of contraception during and after treatment, and a signed acknowledgement form plus a patient reminder card. These measures were reinforced by the EMA retinoid review completed on 22 March 2018.
The 2018 EU review found the evidence on depression conflicting and could not identify a clear increase in risk, but added a precautionary warning: patients should be monitored for changes in mood or behaviour.
Dryness of the lips, skin, eyes and nose is the most common effect and is dose-dependent.
Can raise liver enzymes and blood fats (triglycerides and cholesterol), so blood monitoring is part of standard EU prescribing.
Authorised only for severe forms of acne — nodular or conglobate acne, or acne at risk of permanent scarring — that has failed to respond to standard courses of systemic antibacterials.
