IN PLAIN TERMS
Flibanserin, sold in the United States as Addyi, is a tablet taken every night at bedtime for premenopausal women whose low sexual desire causes them real distress. It is not a hormone and it is not the female version of an erection tablet: it slowly adjusts brain chemical signalling and can take weeks before any effect shows. It has never been authorised anywhere in the European Union, so no EU doctor can prescribe it and Hi-Doctor does not offer it. Its best-known safety problem is alcohol: drinking close to a dose can cause a sharp drop in blood pressure and fainting, which is why the US label carries a boxed warning and rules about waiting at least two hours after a drink.

The trial ran for 24 weeks in North America and the difference from placebo amounted to roughly one extra satisfying sexual event per month; the placebo group also improved substantially.

NAMED SAFETY SIGNALS
The US label carries a boxed warning that taking flibanserin and alcohol close together in time increases the risk of severe low blood pressure and fainting. At first approval in 2015 alcohol was an outright contraindication; the warning was revised in 2019 to tell patients to wait at least two hours after one or two standard drinks before taking the tablet.
The US label contraindicates flibanserin with moderate or strong CYP3A4 inhibitors and in anyone with liver impairment, because severe hypotension and syncope can occur.
Reported by 11.4% of premenopausal women on flibanserin versus 2.2% on placebo.
Somnolence reported by 11.2% of premenopausal women on flibanserin versus 2.9% on placebo.
Reported by 10.4% of premenopausal women on flibanserin versus 3.9% on placebo.
Syncope was recorded in 0.4% of premenopausal women on flibanserin versus 0.2% on placebo, and hypotension in 0.2% versus less than 0.1%.
In the BEGONIA trial 9.6% of women on flibanserin discontinued because of adverse events, versus 3.7% on placebo.
