What Duac actually does

Duac combines two complementary active ingredients: clindamycin (1%) and benzoyl peroxide (available in 3% or 5% formulations). Clindamycin is a lincosamide antibiotic that suppresses the growth of Cutibacterium acnes, reducing follicular inflammation. Benzoyl peroxide acts as a broad-spectrum oxidizing antibacterial agent with mild keratolytic properties.

The benzoyl peroxide component prevents C. acnes from developing resistance to clindamycin, which is why topical clindamycin should never be prescribed as a monotherapy. Therapy is capped at a strict maximum of 12 weeks to minimize the risk of bacterial resistance and cutaneous irritation. Retinoid or antimicrobial purging may occur in the first few weeks, and meaningful clinical improvement is formally evaluated at 12 weeks.

Side effects and when to stop

Topical side effects are common during the initial weeks of therapy as the skin adapts to benzoyl peroxide and clindamycin.

  • Common reactions: Application site erythema, dryness, peeling, mild burning, and localized stinging.
  • Fabric bleaching and sun sensitivity: Benzoyl peroxide bleaches hair, clothing, and bedding on contact. It also increases photosensitivity, making daily broad-spectrum sunscreen essential.
  • When to stop immediately: Discontinue treatment and seek medical attention if you experience severe abdominal cramps, prolonged watery or bloody diarrhea, or signs of an acute hypersensitivity reaction.
Important notice: Hi-Doctor provides medical consultations and digital prescriptions only. We do not sell, ship, or deliver medication. Approved REMPE prescriptions can be presented at any EU pharmacy, where the pharmacist reviews the prescription and may dispense the medication. Pharmacists retain the right to refuse dispensing.
