If you want to get Duac online in Germany, you can request an evaluation through a secure medical questionnaire without waiting months for a dermatologist appointment. Duac gel combines clindamycin 1% with benzoyl peroxide (3% or 5%) to treat mild to moderate inflammatory acne. An EU-licensed doctor reviews your clinical history and skin photos asynchronously. When clinically appropriate, the doctor issues an official REMPE electronic prescription in under 24 hours. The 40 EUR consultation fee is refunded in full if the doctor does not issue a prescription. You can download the prescription PDF and present it at local pharmacies in Germany, where the pharmacist may dispense the medication.

Over 50,000 patients have accessed asynchronous medical consultations through Hi-Doctor, supported by a 4.8/5 Trustpilot rating.

How it works — get your consultation in 4 steps

Getting assessed for prescription acne treatment takes only a few minutes online. The entire clinical pathway is asynchronous, private, and GDPR-compliant.

  1. Complete the medical questionnaire: Answer questions about your skin type, previous acne treatments, medical history, and upload clear photographs of the affected areas.
  2. Doctor clinical assessment: An EU-registered doctor reviews your responses and photographs within 24 hours to determine if topical Duac is safe and clinically appropriate for you.
  3. Receive your electronic prescription: If approved, an official REMPE electronic prescription PDF is delivered straight to your secure Hi-Doctor account and transactional email. If the doctor determines treatment is not suitable, the 40 EUR consultation fee is refunded in full.
  4. Present the prescription at a pharmacy: Download or print the PDF to present at any pharmacy in Germany or across the EU, where the pharmacist reviews the document and may dispense the medication.

Who qualifies

Duac gel is a prescription-only topical treatment formulated for specific types of acne. Online consultations are designed to identify whether this specific combination therapy matches your clinical presentation.

Editorial still life illustrating Who qualifies
Who qualifies
  • Eligible conditions: Adults and adolescents aged 18 and older experiencing mild to moderate inflammatory acne vulgaris characterised by papules and pustules.
  • Photo review required: You must provide well-lit, clear photographs so the doctor can assess lesion types, distribution, and skin integrity accurately.
  • Out of scope for online care: Severe nodulocystic acne, conglobata, and deep scarring cannot be treated through this service. Hi-Doctor does not prescribe oral isotretinoin or oral antibiotics online; patients with severe disease are referred to in-person specialist care.
  • Contraindications: Patients with known hypersensitivity to clindamycin, lincomycin, or benzoyl peroxide, or those with a history of enteritis, ulcerative colitis, or antibiotic-associated colitis, do not qualify.

What Duac actually does

Duac contains two complementary active ingredients: clindamycin (1%) and benzoyl peroxide (3% or 5%). Clindamycin is a lincosamide antibiotic that halts the synthesis of bacterial proteins, effectively suppressing Cutibacterium acnes populations inside blocked pores.

Benzoyl peroxide acts as a potent oxidising agent with keratolytic and antibacterial properties. It prevents C. acnes from developing resistance to clindamycin, which is why topical clindamycin is never prescribed as monotherapy. Duac is prescribed for once-nightly application and is strictly capped at a maximum duration of 12 weeks to minimise bacterial resistance risks.

Side effects and when to stop

Most patients experience mild, localised skin reactions during the first few weeks of using Duac gel. Common side effects include erythema (redness), peeling, skin dryness, and mild stinging or burning at the application site.

Benzoyl peroxide bleaches fabrics and dyed hair. Apply carefully at bedtime and wash hands thoroughly after application.

Discontinue treatment immediately and seek emergency medical care if you develop signs of an allergic reaction (swelling of the face, lips, or throat, or severe hives). If you experience severe or prolonged abdominal cramps or watery diarrhea, stop using the gel immediately, as systemic absorption of clindamycin—though rare—can induce antibiotic-associated colitis.
