Minoxidil prolongs the anagen (growth) phase of the hair follicle and increases hair diameter. Topical is the standard in mild-to-moderate androgenetic alopecia; low-dose oral (off-label, under medical supervision) is an alternative when adherence to topical is poor or the response is insufficient.

It's frequently combined with a 5-alpha-reductase inhibitor: minoxidil stimulates the follicle while finasteride or dutasteride blocks the hormonal cause. The doctor decides the form and the combination based on your history and pattern of loss.

Hoe werkt minoxidil bij haar?
Minoxidil verlengt de anagene fase (groeifase) van het haarzakje en maakt de haren dikker. Het wordt uitwendig gebruikt of, onder medisch toezicht, oraal in een lage dosis als uitwendig gebruik onvoldoende werkt.
Uitwendig gebruik is de standaard bij lichte tot matige androgenetische alopecia. Oraal in een lage dosis (off-label) is een alternatief als het lastig is om het uitwendige middel vol te houden of als het effect onvoldoende is. Elke vorm heeft een eigen bijwerkingenprofiel.

Ja, dat is een veelgebruikte combinatie. Minoxidil stimuleert het haarzakje en de 5-alfa-reductaseremmer blokkeert de hormonale oorzaak. De arts beslist op basis van je voorgeschiedenis en het patroon van je haaruitval.
Extra haaruitval in het begin is te verwachten en wordt beschreven in de productinformatie. Die begint meestal twee tot zes weken na de start en neemt binnen ongeveer twee weken af. De reden is dat minoxidil rustende haarzakjes terugbrengt naar de groeifase, en de oude haren in die haarzakjes loslaten wanneer de nieuwe doorkomen. Het is een teken dat de haarzakjes reageren, geen teken dat het geneesmiddel je haar beschadigt. Houdt sterke haaruitval langer dan ongeveer twee weken aan, stop dan en overleg met de arts die je behandeling begeleidt.
Minoxidil houdt haar alleen in stand zolang je het blijft gebruiken. Volgens de productinformatie kan teruggegroeid haar drie tot vier maanden na het stoppen verdwijnen, en gaat het kaalheidspatroon dat je eerder had daarna verder waar het was gebleven. Daarom beschrijven de instructies van de fabrikant tweemaal daags aanbrengen als nodig voor blijvende groei, en niet als een vaste kuur. Hetzelfde geldt voor de orale vorm. Maken bijwerkingen of de dagelijkse routine het moeilijk om vol te houden, bespreek dat dan met de voorschrijver, die kan beoordelen of de behandeling nog zinvol voor je is.
Dat kan alleen een voorschrijver beslissen, en het is off-label gebruik. Minoxidiltabletten zijn in de hele EU geregistreerd voor ernstig hoge bloeddruk, niet voor haaruitval, en de bijsluiter als bloeddrukverlager waarschuwt voor vocht rond het hart, gezien bij ongeveer 3% tot 5% van de behandelde patiënten die geen dialyse krijgen. In een reeks van 1.404 patiënten die een lage orale dosis voor hun haar gebruikten, was ongewenste lichaamsbeharing met 15,1% het meest voorkomende probleem, met een licht gevoel in het hoofd bij 1,7% en vochtophoping bij 1,3%. Een arts weegt je voorgeschiedenis van hart, nieren en bloeddruk af voordat het wordt overwogen.

Dat hangt af van wat je gebruikt. Vermeld in de vragenlijst elk geneesmiddel, supplement en kruidenmiddel, ook als je het maar af en toe gebruikt. De arts controleert op wisselwerkingen voordat er wordt voorgeschreven, en je apotheker controleert dat opnieuw wanneer je Regaine ophaalt.
De arts kan een andere behandeling tegen haaruitval overwegen, een andere vorm (tablet of oplossing) of een combinatie, afhankelijk van het patroon van je haaruitval en je voorgeschiedenis. Sommige situaties, zoals plotselinge of pleksgewijze haaruitval, vragen om een dermatoloog die je persoonlijk ziet, en dat vertelt de arts je dan.

Hoe werkt Regaine?

Minoxidil opens potassium channels. Your body converts minoxidil into minoxidil sulfate, its active form. That form opens ATP-sensitive potassium channels in the smooth muscle lining small blood vessels. Potassium flows out of the cell, the muscle relaxes, and the vessel widens. This is the same action that lowers blood pressure in tablet form.

Blood flow around follicles rises. Wider vessels mean more microcirculation reaching each hair follicle. Minoxidil also raises vascular endothelial growth factor, a signal the manufacturer's product information says is probably responsible for the extra openings in nearby capillaries. Those openings feed a follicle while it is actively growing and burning energy.

The growing phase lasts longer. Follicles then stay longer in anagen, the active growing phase, leave the resting phase sooner, and produce a thicker hair shaft. How minoxidil produces this effect in hair is still not fully established. The product information describes what happens rather than one proven pathway.

Hoe gebruik je Regaine?

Start with a dry scalp. Hair and scalp must be thoroughly dry before you apply minoxidil. Damp skin absorbs more of it than intended, which raises the chance of dizziness, a faster heartbeat and other whole-body effects.

Measure one dose exactly. One dose is 1 ml of solution, which is six presses of the pump, or 1 g of foam, roughly half a cap. Do not exceed 2 ml or 2 g in any 24 hours.

Spread across thinning areas. Apply to the centre of the thinning area on the scalp, then spread outwards with your fingertips. It goes on the scalp only, never on other parts of the body.

Wash your hands afterwards. Wash your hands thoroughly straight after applying. Minoxidil carried from hands, pillows or towels onto the face, or onto another person, can grow unwanted hair there, and that is a commonly reported effect.

Let it dry completely. Give the scalp time to dry before styling, putting on a hat or going to bed. Do not cover a treated scalp with dressings or other occlusive coverings, which push more of the drug through the skin.

Use it twice every day. The 5% products for men are applied twice daily, morning and evening. Missed applications reduce the effect, because the benefit depends on continuous exposure rather than on building up a reserve in the body.

Oral tablets: one daily dose. If a doctor prescribes minoxidil tablets for hair loss, that is an off-label use of a blood-pressure medicine. Take the exact dose prescribed, at the same time each day, and never adjust it yourself.

Keep going to keep results. Minoxidil holds hair only while you use it. Regrown hair can disappear three to four months after stopping, and normal balding then resumes. Tell the prescriber before you stop rather than stopping quietly.

Gebruik Regaine niet als

  • You are allergic to minoxidil, to ethanol (alcohol) or to propylene glycol.
  • You have high blood pressure, whether or not it is being treated.
  • Your scalp is not healthy: psoriasis, sunburn, cuts, infection or inflammation.
  • You have shaved your scalp, which increases how much minoxidil is absorbed.
  • You are using other medicated scalp products, or occlusive dressings on the scalp.
  • You are pregnant, planning a pregnancy, or breastfeeding.
  • You have phaeochromocytoma, a tumour of the adrenal gland (oral minoxidil).
  • You are under 18 or over 65, ages at which the topical products are not recommended.
  • Your hair loss is sudden, patchy, or has no clear cause, rather than pattern hair loss.
  • You have heart disease or a heart-rhythm problem and have not spoken to a doctor first.

Geneesmiddelen en stoffen die een wisselwerking geven

  • Other blood-pressure medicines: Blood-pressure lowering adds up, so pressure can fall further than intended. The prescriber reviews every blood-pressure medicine before considering oral minoxidil.
  • Guanethidine or betanidine: Reported to cause excessive blood-pressure drops and dizziness on standing. Guanethidine is stopped before oral minoxidil is started.
  • Other peripheral vasodilators: May add to dizziness on standing, a theoretical risk even with topical minoxidil. The prescriber checks for overlapping vasodilators and reviews symptoms at follow-up.
  • Other medicated scalp treatments: Should not be applied to the scalp at the same time as minoxidil. The prescriber sequences scalp treatments instead of layering them together.
  • Topical corticosteroids, tretinoin, dithranol or petrolatum: Alter the skin barrier and can increase how much minoxidil is absorbed. Kept off the treated scalp unless a doctor has specifically agreed to it.
  • Anti-inflammatory painkillers and calcium channel blockers: May add to the fluid retention and ankle swelling seen with oral minoxidil. The prescriber watches for swelling and reviews whether the combination continues.
