Tirzepatide is a dual receptor agonist. It acts on the GLP-1 and GIP gut-hormone receptors at the same time. In clinical trials this translates into larger average weight loss than mono-agonist GLP-1s in adults with obesity.

It's a chronic treatment for as long as it's beneficial and safe: the doctor reviews your response every 3 months and decides with you whether to continue, adjust or stop. Discontinuation is gradual and paired with a maintenance plan.

Wat is het verschil tussen tirzepatide en een klassieke GLP-1?
Tirzepatide is een duale receptoragonist (GLP-1 en GIP) die in studies bij volwassenen met obesitas gemiddeld meer gewichtsverlies laat zien dan GLP-1-middelen die op één receptor werken.
Niet per se. Het is een langdurige behandeling zolang die zinvol en veilig is: de arts beoordeelt de respons elke 3 maanden en beslist samen met jou of je doorgaat, aanpast of stopt. Stoppen moet geleidelijk gebeuren en samengaan met een plan om het resultaat vast te houden.

Ja. Tirzepatide is de werkzame stof, en Mounjaro is de merknaam waaronder het in de Europese Unie wordt verkocht. In de Verenigde Staten wordt hetzelfde molecuul voor gewichtsbeheersing ook verkocht als Zepbound, maar Zepbound is niet toegelaten in de EU, dus een EU-recept voor tirzepatide wordt uitgeschreven voor Mounjaro. Het Europees Geneesmiddelenbureau liet Mounjaro toe op 15 september 2022, en de vergunning omvat nu zowel diabetes type 2 als gewichtsbeheersing bij volwassenen die aan de BMI-criteria voldoen.
Het gewicht komt meestal terug. In SURMOUNT-4 werden volwassenen die tijdens een aanloopfase van 36 weken al gemiddeld 20,9% waren afgevallen, willekeurig ingedeeld: ze bleven tirzepatide gebruiken of stapten over op placebo. Tussen week 36 en week 88 veranderde het gewicht in de groep die doorging met nog eens -5,5%, terwijl de placebogroep 14,0% terugkreeg. In week 88 had 89,5% van de groep die doorging minstens 80% van het in de aanloopfase verloren gewicht behouden, tegenover 16,6% van de placebogroep. Stoppen is een beslissing die je samen met je voorschrijver neemt.
Nee. De productinformatie noemt 5 mg, 10 mg en 15 mg als aanbevolen onderhoudsdoses, dus 5 mg en 10 mg zijn op zichzelf geregistreerde einddoses en niet alleen tussenstappen. In SURMOUNT-1 was de gemiddelde verandering van het lichaamsgewicht na 72 weken -15,0% in de groep met 5 mg, -19,5% in de groep met 10 mg en -20,9% in de groep met 15 mg. Maag- en darmbijwerkingen kwamen bij hogere doses ook vaker voor. Je voorschrijver bepaalt waar je stopt met opbouwen, op basis van hoe je reageert en hoe goed je elke stap verdraagt.

Sommige mensen zijn beter geholpen bij een obesitaskliniek waar ze persoonlijk terechtkunnen of bij hun eigen arts, en de arts legt uit waarom als dat zo is.

Hoe werkt Tirzepatide?

Copies two gut hormones. Tirzepatide activates the receptors for two natural gut hormones, GIP and GLP-1, which the body releases after eating. The EU product information describes it as a long acting GIP and GLP-1 receptor agonist that binds both receptors with high affinity.

Turns down appetite signals. Both of those receptors also sit in the brain areas that regulate appetite. Clinical studies cited in the product information report that tirzepatide reduces energy intake and appetite by increasing feelings of satiety and fullness, and by decreasing feelings of hunger.

Slows the stomach, steadies glucose. Tirzepatide delays gastric emptying, so food leaves the stomach more slowly after a meal. It also increases insulin release in a glucose dependent way and lowers glucagon. The weight change comes mainly from eating less, and is mostly a loss of fat mass.

Hoe gebruik je Tirzepatide?

Pick one day each week. Tirzepatide is injected once a week, at any time of day, with or without food. Pick a day that is easy to remember. You can change the day later as long as three days separate two doses.

Store pens in the fridge. Unused pens are kept in a refrigerator between 2 and 8 degrees Celsius, in the original carton so light cannot reach them, and must never be frozen. A pen that has been frozen must not be used.

Check the pen before use. Look at the liquid before you inject. It should be clear and free of particles or discolouration. Discard the pen if you can see anything floating in it, if it looks discoloured, or if it is out of date.

Inject into tummy, thigh or arm. Tirzepatide goes under the skin of the abdomen, the thigh or the upper arm. The back of the upper arm needs another person to do the injection. Read the instructions for use packed with the pen first.

Rotate the spot every week. Injection sites should be rotated with each dose, so the same patch of skin is not used week after week. If you also inject insulin, use a different site for each of the two medicines.

Missed dose: the four day rule. If you remember within four days, inject then and return to your usual schedule. If more than four days have passed, skip that dose and take the next one on the normal day. Never inject two doses to catch up.

Know how long a pen lasts. Single-dose pens and vials may be kept out of the refrigerator for up to 21 cumulative days below 30 degrees Celsius, then discarded. A multi-dose KwikPen holds four weekly doses and is thrown away 30 days after first use.

Keep the diet and activity going. Tirzepatide is licensed as an addition to a reduced-calorie diet and increased physical activity, not as a replacement for them. Every trial result quoted on this page comes from people doing both alongside the weekly injection.

  • You have ever had an allergic reaction to tirzepatide or to any other ingredient in the pen.
  • You are pregnant or planning a pregnancy. Tirzepatide is not recommended in pregnancy or in women of childbearing potential not using contraception.
  • You are breastfeeding. A risk to the infant cannot be excluded, so the prescriber weighs continuing feeding against continuing treatment.
  • You have had pancreatitis. Tirzepatide has not been studied in these patients and should be used with caution.
  • You have severe gastrointestinal disease, including severe gastroparesis. Tirzepatide has not been studied in this group.
  • You have proliferative diabetic retinopathy, diabetic macular oedema, or retinopathy needing acute treatment. Caution and monitoring apply.
  • You are under 18 years old. Safety and efficacy for weight management have not been established below that age.
  • You have severe kidney impairment, end stage renal disease, or severe liver impairment. Experience is limited, so caution applies.
  • You are booked for general anaesthesia or deep sedation. Tell the team you take tirzepatide, because pulmonary aspiration has been reported.

Geneesmiddelen en stoffen die een wisselwerking geven

  • Oral medicines in general: Tirzepatide delays gastric emptying, so tablets can be absorbed more slowly, especially at the start. Most oral medicines need no dose change. The prescriber reviews your full list.
  • Warfarin, digoxin and other narrow therapeutic index drugs: Absorption can shift when tirzepatide is started or the dose is increased. The prescriber monitors you more closely at initiation and after each dose increase.
  • Sulfonylureas (for example gliclazide): Combined use raises the risk of low blood sugar in people with type 2 diabetes. The prescriber may reduce the sulfonylurea dose and ask for blood glucose self-monitoring.
  • Insulin: Combined use raises hypoglycaemia risk; clinically significant lows occurred in 14 to 19 percent. The prescriber reduces insulin stepwise and arranges blood glucose self-monitoring.
  • Combined oral contraceptives: A single 5 mg dose reduced ethinylestradiol peak level by 59 percent and delayed it four hours. EU labelling requires no dose change. UK labelling advises adding a barrier method for four weeks.
  • Paracetamol: A single 5 mg dose halved the paracetamol peak level and delayed it by one hour. No dose adjustment needed. Overall exposure was unchanged after four consecutive weekly doses.
  • Oral medicines where rapid onset matters: Delayed gastric emptying can postpone the effect of a tablet you need to work quickly. The prescriber considers timing, a non-oral route, or an alternative medicine.
