Semaglutide is a GLP-1 receptor agonist, a medication that mimics a natural hormone your gut releases after eating. It reduces appetite, slows gastric emptying and helps maintain sustained weight loss when combined with supervised nutrition and activity changes.

It's prescribed alongside changes to diet and activity, as part of a medically supervised plan, not on its own, and not as a shortcut. A licensed EU doctor decides whether it's appropriate and safe for you.

Wat is semaglutide en hoe ondersteunt het gewichtsbeheersing?
Semaglutide is een GLP-1-receptoragonist die een natuurlijk darmhormoon nabootst. Het vermindert de eetlust, vertraagt de maaglediging en helpt blijvend gewichtsverlies vast te houden in combinatie met begeleide aanpassingen in voeding en beweging.
In de EU is het geïndiceerd als aanvullende behandeling bij volwassenen met een BMI ≥ 30, of ≥ 27 met metabole bijkomende aandoeningen. Het is geen cosmetische behandeling: de bevoegde arts bekijkt je voorgeschiedenis, huidige medicatie en doelen voordat wordt besloten of het veilig voor je is.

Lichte misselijkheid, een opgeblazen gevoel en verstopping, vooral bij de start of bij het verhogen van de dosis. Meestal verdwijnen ze binnen 1 tot 2 weken. Ernstige effecten zijn zeldzaam; je arts legt uit op welke alarmsignalen je moet letten en hoe je hem of haar bereikt via de privéchat in de app.
De dosis wordt in 16 weken opgebouwd. Veranderingen in eetlust treden meestal op na 2 tot 4 weken; klinisch betekenisvol gewichtsverlies wordt na 3 maanden beoordeeld. Onze artsen doen elk kwartaal een controle voordat het recept wordt verlengd.
Het gewicht komt meestal terug. Dat werd rechtstreeks gemeten in de STEP 4-studie. Alle deelnemers bouwden eerst in 20 weken op naar de dosis van 2,4 mg, waarna de helft doorging en de andere helft overstapte op een nepinjectie. In de 48 weken daarna verloor de groep die doorging gemiddeld nog 7,9% van het lichaamsgewicht, terwijl de groep die stopte 6,9% terugkreeg. Dat is een verschil van 14,8 procentpunten. Semaglutide wordt daarom gebruikt als langdurige behandeling en niet als korte kuur, en elke beslissing om te stoppen hoort besproken te worden met de voorschrijvende arts.
De volledige dosis wordt pas in week 17 bereikt. De productinformatie beschrijft een bewuste opbouw van 16 weken, beginnend met 0,25 mg en elke vier weken een stap hoger, alleen om misselijkheid en andere maagklachten te beperken. Sommige mensen merken in die eerste weken al minder eetlust, maar de onderhoudsdosis van 2,4 mg begint pas daarna. De belangrijkste STEP 1-studie mat het hoofdresultaat na 68 weken, ongeveer 16 maanden, dus het gaat om maanden en niet om weken. Je voorschrijver kan de opbouw verder vertragen als bijwerkingen lastig zijn.
De NHS raadt aan alcohol te vermijden, omdat het bijwerkingen zoals misselijkheid of overgeven kan verergeren. Misselijkheid is de meest gemelde bijwerking van semaglutide, bij meer dan één op de tien mensen, en is het duidelijkst tijdens de opbouw van de dosis, als de maag zich al aanpast. Alcohol voegt ook calorieën toe die een caloriearm dieet tegenwerken, en kan de bloedsuiker beïnvloeden, wat zwaarder weegt als je ook insuline of een sulfonylureumderivaat gebruikt. Drink je regelmatig, bespreek dat dan met de voorschrijvende arts in plaats van het zelf te beslissen.
Schrijf je arts via de privéchat in je account: beschrijf wat je merkt en wanneer het begon. De arts kan het plan aanpassen, onderbreken of een andere optie voorstellen. Heb je ernstige klachten zoals moeite met ademhalen, zwelling van het gezicht of pijn op de borst, bel dan direct 112 of ga naar een spoedeisende hulp.
Dat hangt af van wat je gebruikt. Vermeld in de vragenlijst elk geneesmiddel, supplement en kruidenmiddel, ook als je het maar af en toe gebruikt. De arts controleert op wisselwerkingen voordat er wordt voorgeschreven, en je apotheker controleert dat opnieuw wanneer je Semaglutide ophaalt.
De arts kan een ander in de EU toegelaten geneesmiddel voor gewichtsbeheersing overwegen, of een plan dat alleen op voeding en beweging is gebaseerd. Sommige mensen zijn beter geholpen bij een obesitaskliniek waar ze persoonlijk terechtkunnen of bij hun eigen arts, en de arts legt uit waarom als dat zo is.
Voor elke verlenging bekijkt de arts je gewicht, bijwerkingen en hoe je je voelt, en beslist of je doorgaat, de dosis verandert of stopt. Wijzigingen gebeuren nooit automatisch. Tussen de beoordelingen door kun je ook schrijven in de privéchat.

Het is een pdf met alle gegevens die EU-richtlijn 2011/24/EU vereist (patiënt, voorschrijver met land van registratie, werkzame stof, dosis), waardoor het wettelijk erkend is in alle 27 EU-lidstaten. In Spaanse apotheken wordt het via de QR-code verstrekt; buiten Spanje verstrekt de apotheker het lokaal verkochte equivalent met dezelfde werkzame stof.

Het plan kost € 25/maand voor gewichtsverlies of € 15/maand (gefactureerd € 45 per 90 dagen) voor haargroei; een eenmalig consult over seksuele gezondheid kost € 35 en een huidconsult voor acne, rosacea, melasma en huidveroudering kost € 40. Hoe werkt Semaglutide?

It copies a natural gut hormone. Semaglutide is a close copy of GLP-1, a hormone your gut releases after you eat. It shares about 94% of the structure of the natural hormone, so it fits the same docking points on cells, but it is built to last much longer in the body.

It acts on appetite centres. Once attached to those docking points, semaglutide signals the parts of the brain that control hunger, including the hypothalamus and brainstem. In clinical studies people reported feeling full sooner, staying full for longer, feeling less hungry, and having fewer and weaker food cravings.

Eating less drives the change. Because appetite falls, most people take in fewer calories without consciously trying, and that gap is what produces weight change over months. Semaglutide also slightly slows how fast the stomach empties early after a meal, and prompts insulin release only when blood sugar is high.

Hoe gebruik je Semaglutide?

One injection, same day weekly. Semaglutide for weight management is injected under the skin once a week, at any time of day, with or without food. Most people pick a fixed day and set a reminder, because the weekly rhythm is easy to lose track of.

Choose and rotate injection sites. The approved sites are the abdomen, the thigh and the upper arm. The site can be changed from week to week. Rotating between sites is standard practice for injections under the skin and helps avoid repeated irritation in one spot.

Use a fresh needle every time. Attach a new needle for each injection and remove it afterwards, storing the pen without a needle attached. The product information says this helps prevent blocked needles, contamination, infection, leaking solution and inaccurate dosing. Compatible needles are 30G, 31G or 32G.

Check the liquid before injecting. Look at the solution first. It should be clear and colourless. The product information says not to use the pen if the liquid looks otherwise, or if it has been frozen at any point. Pens must never be shared between people.

Follow the pack instructions exactly. Each pen type has its own instructions for use inside the carton, and the product information asks patients to read them carefully before the first injection. With a single-dose pen you press it firmly against the skin until the yellow bar stops moving.

If you miss a dose. Take a missed dose as soon as possible, as long as fewer than five days have passed. If more than five days have gone by, skip it and take the next dose on the normal day. Ask the prescriber before restarting after several missed doses.

Moving your weekly dosing day. The day of the week can be changed if needed, provided at least three days, meaning more than 72 hours, separate one dose from the next. After moving to a new day, carry on with the once-weekly pattern from there.

Storing the pen correctly. Before first use, keep the pen in the fridge between 2°C and 8°C, away from the cooling element, and never freeze it. Keep the cap on to protect it from light. After first use a FlexTouch pen keeps for six weeks below 30°C.

Gebruik Semaglutide niet als

  • You have ever had an allergic reaction to semaglutide or any ingredient in the pen
  • You are pregnant, planning a pregnancy within the next two months, or breastfeeding
  • You have type 1 diabetes, because semaglutide is not a substitute for insulin
  • You already take another GLP-1 receptor agonist, as the combination has not been evaluated
  • You take another prescription weight-management medicine at the same time
  • You have severe kidney impairment, including end-stage kidney disease
  • You have severe liver impairment
  • You have severe gastroparesis, a condition where the stomach empties very slowly
  • You have congestive heart failure classed as New York Heart Association class IV
  • You have type 2 diabetes with uncontrolled or unstable diabetic eye disease

Geneesmiddelen en stoffen die een wisselwerking geven

  • Warfarin and other coumarin blood thinners: Cases of a reduced INR clotting measure have been reported alongside semaglutide. Prescriber arranges more frequent INR blood tests when semaglutide is started.
  • Insulin: Combined with semaglutide in type 2 diabetes, the risk of low blood sugar rises. Prescriber may lower the insulin dose at the start and monitor blood sugar.
  • Sulfonylureas (e.g. gliclazide): Also raise the risk of low blood sugar when taken alongside semaglutide. Prescriber may reduce the sulfonylurea dose when semaglutide is introduced.
  • Oral medicines needing fast absorption: Semaglutide slows stomach emptying, which could in principle affect how they are absorbed. Prescriber uses caution and reviews whether timing or monitoring needs changing.
  • Combined oral contraceptives: No clinically relevant change; levonorgestrel exposure rose 20%, ethinylestradiol was unaffected. No dose change needed. Contraception is advised while taking semaglutide.
  • Paracetamol: Peak levels fell 23% with semaglutide 1 mg, but total exposure was unchanged. No dose adjustment of paracetamol is required.
  • Atorvastatin: Peak level fell 38% but overall exposure was unchanged, judged not clinically relevant. No dose adjustment is required.
  • Digoxin and metformin: Semaglutide did not change overall exposure or peak levels of either medicine. No dose adjustment is required.
