Ce qu’est Vitaros et comment obtenir un traitement

L’alprostadil reproduit la prostaglandine E1, une substance naturellement produite par le corps qui élargit les vaisseaux sanguins. Contrairement aux comprimés, il est appliqué directement sur le pénis : sous forme de crème sur l’extrémité, d’injection sur le côté du corps du pénis ou de petit bâtonnet placé dans l’urètre. Il agit donc directement sur les vaisseaux sanguins. Il est autorisé dans les pays de l’UE contre les troubles de l’érection depuis 1994 sous forme d’injection et depuis 2014 sous forme de crème. Comme il agit localement, ses principaux effets indésirables sont aussi locaux : brûlure, douleur ou rougeur à l’endroit où il est appliqué et, rarement, une érection qui dure trop longtemps et nécessite des soins médicaux urgents.

Vitaros est autorisé dans l’UE depuis le 29 novembre 1994. Un médecin habilité peut donc vous le prescrire si ce traitement est adapté à votre situation médicale.

The IIEF changes are small in absolute terms and the authors describe the SEP2 and SEP3 improvements as slight though statistically significant; the studies ran for 12 weeks and cover the topical cream only, not the injected or intraurethral forms.

Application-site burning, penile pain and redness (cream)

Penile burning, penile pain, penile erythema, genital pain and genital discomfort are all listed as common in the EU product information for alprostadil 3 mg/g cream. In the clinical trial the withdrawal rate was 30% in the 300 microgram group compared with 20% in the 200 microgram group.

Injection-site pain (intracavernosal)

Penile pain is listed as very common in the EU product information for alprostadil injection. In the six-month self-injection study, usually mild penile pain occurred in 50% of men at some time but after only 11% of injections.

Prolonged erection and priapism (intracavernosal)

In the EU product information for alprostadil injection, a prolonged erection lasting 4 to 6 hours developed in 4% of patients and priapism (a painful erection lasting more than 6 hours) in 0.4%. In the six-month self-injection study, prolonged erections occurred in 5% of men and priapism in 1%.

Prolonged erection and priapism (cream)

Listed as uncommon in the EU product information for alprostadil cream. Priapism was seen in 1 patient (0.06%) across the two 3-month studies and in 5 patients (0.4%) in the study lasting more than 6 months. An erection lasting more than 4 hours needs immediate medical help, because untreated priapism can permanently damage erectile tissue.

Penile fibrosis (intracavernosal)

Penile fibrosis, including angulation, fibrotic nodules and Peyronie's disease, was reported in 3% of clinical-trial patients in the EU product information for alprostadil injection, rising to about 8% in a self-administration study of up to 18 months. Injection-site haematoma was reported by 3% and bruising by 2%.

Dizziness and fainting

In the phase 3 cream studies, symptomatic low blood pressure (dizziness) and fainting occurred in 0.4%, 0.4% and 0.5% of patients on the 100, 200 and 300 microgram doses. Patients are advised not to drive or do hazardous tasks until they know how they react.

Effects on a female partner

Vulvovaginal burning and vaginitis are listed as common reactions in female partners in the EU product information for alprostadil cream.

Penile pain (intraurethral)

In the transurethral pivotal trial, mild penile pain was the most common side effect, occurring after 10.8% of treatments; in-clinic low blood pressure occurred in 3.3% of men, and no man had priapism or penile fibrosis.

Alprostadil is a prostaglandin E1 that directly dilates the blood vessels of the penis. It's considered when oral PDE5 inhibitors are ineffective or contraindicated.

It's administered as a urethral cream or an intracavernous injection, under medical instruction — each route has its own efficacy and tolerability profile. Correct technique matters, and the doctor explains the protocol and the warning signs.

Rather than blocking an enzyme, alprostadil acts directly on the penile blood vessels, dilating them locally — no PDE5 pathway involved, so it can work where oral options don't.

  1. A copy of prostaglandin E1

    Alprostadil is chemically identical to prostaglandin E1, a signalling substance the body already makes. It is not swallowed. It is placed into or onto the penis itself, so it acts on the erectile tissue at the site rather than circulating through the whole body first.

  2. Cavernosal muscle relaxes

    The drug relaxes the smooth muscle of the corpora cavernosa, the two spongy cylinders that run the length of the penis, and widens the cavernosal arteries feeding them. Blood flow into the erectile tissue rises, and that increase in flow is what produces rigidity.

  3. It does not need arousal

    Because it acts on the tissue directly, alprostadil does not depend on the nitric oxide signal that sexual stimulation produces. In clinic testing it created erections on its own. That is the core difference from tablets, and it is also why an erection can outlast the encounter.

Utiliser Vitaros au quotidien

  1. Get shown the technique first

    Every form of alprostadil has to be demonstrated by a medical professional before you use it yourself. For the injection and the urethral pellet the first doses are given or supervised in a clinic, not at home, and that step cannot be skipped.

  2. Empty your bladder first

    Urinate before using the cream or the pellet. A moist urethra helps the medicine dissolve and spread. Do not urinate straight after the cream, because the dose can be washed out before it has had time to work.

  3. Cream sits at the opening

    Roll the single-dose container between your hands to warm it, hold the penis upright and squeeze the tip to widen the opening. Place the container at the opening without inserting it, push all the cream in, then hold the penis upright for about 30 seconds.

  4. Pellet needs a stretch and roll

    For the urethral pellet, stretch the penis upward to its full length, insert the applicator, press the button and withdraw it. Then roll the penis between your hands for at least 10 seconds so the medicine coats the urethral wall.

  5. Stand or walk for ten minutes

    After the pellet, sit or preferably stand or walk for about 10 minutes while the erection develops. An erection usually appears 5 to 10 minutes after the pellet and lasts roughly 30 to 60 minutes. The cream works in 5 to 30 minutes.

  6. Injections rotate side and site

    The injection goes into the upper side of the penis, along the third nearest the body, avoiding visible veins. Change both the side and the exact spot each time. Never straighten or reuse a bent needle, because needles can break.

  7. Use a condom if pregnancy is possible

    A condom is required if your partner is pregnant or could become pregnant, and it also reduces vaginal irritation, which is a reported reaction. Only latex condoms have been tested with the cream. Alprostadil is not a contraceptive and does not prevent infection.

  8. Respect the frequency limits

    The cream is used at most 2 to 3 times a week and once in 24 hours. Injections are at most once a day and three times a week. The pellet is at most 2 doses in 24 hours and 7 in 7 days.

  • Your partner is pregnant or could become pregnant and a condom will not be used

These are group averages from trials, not a forecast for any one person. Both routes were screened in clinic first, and the home figures describe only the men who had already responded to that supervised test dose, so they read better than the result an unselected group would get. The European Association of Urology also records that 41% to 68% of men stop intracavernous injections, most within the first two to three months, and that only about 30% stay on the urethral pellet long term, which is a fair reminder that tolerating a treatment matters as much as its measured efficacy.

Alprostadil is prescription-only in every EU country and no form of it is sold over the counter anywhere. Which form you can actually obtain varies a lot, so each row was read off that country's own medicines register rather than inferred from the EU-wide picture. The registers do not all publish the same thing: Spain, Portugal and Belgium record a marketing status per product, so those rows describe what is genuinely on sale, while Germany and Austria publish only whether an authorisation is currently valid, so their rows describe which forms are registered rather than which a pharmacy is stocking this week. Poland registers no alprostadil product for erectile dysfunction at all. The linked database is the current authority in every case.
